Our Community Care Coaches provide tailored programmes for each carer through home visits, telephone support and an information pack. The support that is offered includes:

CARE OF PHYSICAL HEALTH:

  • How to avoid a Urinary Tract Infection (UTI)
  • How to reduce the risk of falls
  • How to avoid and minimise the risk of pressure sores and chest infections

NUTRITION & HYDRATION:

  • How to encourage the older person to eat when they have no appetite and to maintain their recommended fluid (drink) intake
  • How to manage weight and prevent obesity
  • Meal planning – menus and ideas on how to produce tasty, healthy food on a limited budget and time

PREVENTING HOSPITAL ADMISSIONS OR THE NEED FOR RESIDENTIAL CARE:

  • Support to develop Carer Emergency Plans
  • Where to get support and knowing when to ask for help – accessing primary, community health and social care services

CARERS WELL BEING:

  • Our menu of services include individual support, short breaks, grants and funding support, an online information hub and other support services where required
  • Strategies for coping and developing emotional resilience
  • Back care and physical health for carer
